Lessons Learned from an Integrated MTM Program

Holyoke Health Center's integrated medication management (IMM) program addresses medication adherence, improves coordination of care, and increases the role of the pharmacist in an integrated care setting. The pharmacy at Holyoke — a community, nonprofit, federally qualified health center in western Massachusetts — is a model for integrated care, with pharmacists playing key roles in decision making and charting.
